How to apply/Instructions

         BGO-F-01 Online Form can reduce time of the submission by filling the form completely and attach a photo copy of the paper BGO-F-01 (scanned paper). When staff received the document, will check, progess and continue update the submission status. Applicants can monitor the status of documents through website.
         Once, when the document has been verified as corrected by staff. The requestor is then must sent original documents and other related documents by postal mail to access the following process.
